Indiana Jr. & Sr. Athletic Information for 2021-22 School year
Sport Physical’s
NO PHYSICAL…NO PARTICIPATION!! Sports physicals are required for all students’ athletes. The sports physical forms (Section 1-7) must be completed and returned to either the senior high school athletic office or the school nurse or the main office at the Jr High. You can also give to your coach to hand into the athletic office. Please turn them in ASAP. PIAA only requires one complete sports physical per school year. Sports physicals for the 2021-2022 school year must be administered by an Authorized Medical Examiner (MD, DO, PAC, CRNP, SNP) and dated June 1, 2021 or later and will be valid until May 31, 2022. Bring the whole physical form with you to the doctor’s office
RE-CERTIFICATION PROCESS
If a student-athlete participated in a fall/winter sport or had a sports physical (Sections 1-7) completed, regardless of when performed during the school year, he/she will not need another sports physical if they will be participating in a winter or spring sport. However, a parent or guardian must complete a Re-certification for Sports Participation form (Section 8) to be eligible to participate in a subsequent sports season (winter and/or spring) in the same school year. The recertification form must be completed not earlier than six weeks prior to the first practice day of the sport season. If any “yes” answers to Section 8, the student-athlete must follow up with a licensed physician and provide documentation to be cleared to participate. INJURED ATHLETE
If a student-athlete participates in a fall, winter or spring sport and is injured, he/she needs to follow up with a licensed physician. Section 8 of the sports physical form will need completed by a parent or guardian and Section 9 must be signed by a licensed physician for clearance before returning to play
Pay-to- Participate
A pay-to-participate has been implemented for students who are participating in certain extra-curricular activities at the Junior and Senior High School. Pay-to-Participate is a per season charge, not a per activity charge. The activities that fall under each season are listed on the Pay-to-Participate form. The fee is: $20 per student per season, regardless of the number of activities the student participates in that season.
- $50 per student for all activities for the school year, must be paid with the fall season.
- $100 per family for the year, regardless of number of children or activities
- The fee is waived for any student who is eligible for free or reduced lunches.
All forms must be sent to the administration office. Payment can either accompany the form or be made on the myschoolbucks.com website. Students who have not paid the fee by the deadline date (see below) will not be eligible to participate until the payment has been received.

NOTE: Sports exempt from the pay to participate fee are:
- Hockey, Boys’ Lacrosse, Girls’ Lacrosse and Boys’ Rugby Club

The most up-to-date schedules are always posted on the website, but there’s always changes throughout the seasons.
ADDITIONAL INFORMATION:
If available, information for each sport will be posted on the athletic website – www.indianahsathletics.bigteams.com as well as forms and much more.
Students are encouraged to contact the head coach for additional information including off-season workouts on that sport.
Eligibility
There are two eligibility checks that all school districts must complete. They are the weekly eligibility and the end of the marking period eligibility. At 9:00 am every Wednesday, the Athletic Department will go on Power School and run a program that determines which students are not passing 4 full credits subjects or are failing 2 classes. At that time, each student who has not met those standards will be emailed by the Athletic Department. The parents and head coach will also receive an email.
The students will have until Friday at 2:00 pm to better his/her grades. The Athletic Department will check Power School on Friday at 2:00 PM to see if the student has brought up their grade. This process gives the students two (2) days to raise one or more grades to passing. If the student does not bring their grade (s) up by 2:00 pm on Friday, he/she will be ineligible for practice and playing sports for the following week (Sunday-to-Saturday). Student, parents, and coaches will then be emailed of the ineligibility after the deadline.
- If a student has not passed four credits at the end of a quarter grading period, the student will be ineligible for 15 school days beginning on the first day the report cards are issued.
- At the end of the school year, the student’s final credits in the student’s subjects rather than the student’s credits for the last grading period shall be used to determine the student’s eligibility for the next grading period.
Attendance
If you are absent from school during a semester for a total of 20 or more school days, you will lose your eligibility until you attend school for a total of 45 school days following your 20th day of absence.
