Sports Season & Physical Information - Grades 7-12
Any student who wishes to participate in athletics during the 2022-2023 academic school year must complete specific documents in order for the student-athlete to be considered eligible for participation.

Student-athletes are permitted to use the PIAA sports physical forms for obtaining their sports physical for the 2022-2023 school year. Sports physicals are required for all students' in grades 7-12 only. The sports physical forms (Section 1-7) must be completed and returned to either the senior high school athletic office or the school nurse. Sports physicals for the 2022-2023 school year must be administered by an Authorized Medical Examiner (MD, DO, PAC, CRNP, SNP)and dated no earlier than June 1, 2022.

Re-certification Forms – (Sections 8 and 9) Following completion of a CIPPE (physical forms), the same student seeking to participate in practice, inter-school practices, scrimmages, and/or contests in subsequent sport(s) in the same school year (WINTER AND SPRING), must complete Section 8 and must turn in that section to the school nurse. Complete Section 8 and include the following: ~Name, date of birth, age, grade and winter or spring sport ~Personal information only if changes ~Emergency information only if changes ~SUPPLEMENTAL HEALTH HISTORY QUESTIONS: YES answers require follow up by a physician with completion of Section 9

A pay-to-participate has been implemented for students who are participating in certain extra-curricular activities at the Junior and Senior High School. Pay-to-Participate is a per season charge, not a per activity charge. The activities that fall under each season are listed on the Pay-to-Participate form.

To reserve the old gym, new gym or turf field time at the Indiana Senior High School, you must submit an Application for Use of School Facilities form. Please contact Jody Lewandowski, Assistant Athletic Director via email at jlewandowski@iasd.cc or phone at 724-463-3530 prior to completing the application to confirm the availability of your request. Once confirmed, complete the form with the name of your organization, type of organization, school building requested (Indiana Sr. High School), room(s)/area(s), date(s), hours (please include the set up time,start time and end time) that you would like to reserve the gym or field. Return the application to Jody Lewandowski in the High School Athletic Office — send it via US mail, email or fax it to 724-463-1394 to her attention. *If school is canceled due to an emergency or weather conditions, all activities are canceled. To reserve classrooms, auditorium,cafeteria or other areas in the Senior High School building, please contact Joyce Shirley, Administrative Assistant in the high school main office. She can be reached at 724-463-8562.
