Two (2) Sport Participation - Same Sports Season Sport Declaration Form must be completed if a student-athlete intends to participate in two (2) PIAA designated sports teams for the current school year in the same sports season that will be indicated by the student-athlete when completing the form.  Forms must be returned to the athletic department.
