Return to Play Clearance
If an athlete is seen by a licensed physician, they must bring a note or authorized form stating their permissible activity level. The note or authorized form may also contain any instructions for the athletic trainer and/or school nurse to follow, such as rehabilitation exercises or modalities with the athlete. This rule applies for EVERYONE regardless of the diagnosis, i.e. concussion, fracture, contusion, sprain, etc. Due to state guidelines, we are unable to accept a return to play clearance from anyone other than a licensed physician (MD, DO., PAC, CRNP or SNP). We cannot accept notes or authorized forms from health care professionals such as chiropractors, registered nurses, medical assistants, etc.
For example, if a fracture is suspected and an x-ray is taken, a note or authorized form that has been SIGNED by the licensed physician (MD, DO., PAC, CRNP or SNP), must be produced stating if/when the athlete is cleared to return to participation and if/when the athlete is allowed to participate in contact drills/full participation in sports.
