Student-athletes are permitted to use the PIAA sports physical forms for obtaining their sports physical for the 2020-2021 school year. Sports physicals are required for all students' in grades 7-12 only. The sports physical forms (Section 1-6) must be completed and returned to either the high school main office, senior high school athletic office or the school nurse. Sports physicals for the 2020-2021 school year must be administered by an Authorized Medical Examiner (MD, DO, PAC, CRNP, SNP) and dated no earlier than June 1, 2020.
